To provide your company with an accurate health insurance quote, a current employee census is required. Please follow the instructions below to provide this information:

1.	Download the Excel Census Document below, OR submit an existing census to ProInsurance if you already have one available.
2.	Email the completed census document to info@proinsurance.com, or fax to 650-326-7400.
3.	A Group Sales Associate will contact you within 24 hours to schedule a time to discuss your company’s savings opportunities.
